Initiate your child’s transformational therapy journey at SOTSBA by following the below steps:

1. Obtain Referral

Begin by obtaining a referral from a primary care physician or specialist to ensure your child's needs align with SOTSBA's services. A referral is necessary for OT, PT, and ABA services, and is highly recommended for ST.

2. Contact Us

Contact us to discuss your needs, challenges, and goals. Call us or request a visit and we'll get back to you within one business day.

3. Verify Insurance

Once initial contact is made, we will assist you in confirming eligibility and benefits with your insurer. You can also visit our Insurance page to view the insurers we are in-network with.

4. Complete Intake Packet

Next, complete and sign the appropriate intake packet so our team can better understand your child’s medical history and how we can provide the best possible service. Please email the complete intake packet to stoneoaktherapy@gmail.com.

5. Schedule Initial Evaluation

With paperwork completed and insurance verified, we will provide available times to schedule your child’s initial evaluation with the appropriate specialist.

6. Create Recurring Schedule

Following the initial evaluation, our specialists will collaborate with you on goals, programs, and skills to create a recurring therapy schedule that works best for you and your child.
